How to fill it out?
Purpose of the form
This form is intended for requesting content removal / access restriction under the Intellectual Property Policy based on an alleged copyright (FSEK) or trademark (SMK) infringement. It must be completed by the rights holder or an authorized representative. This form is not used for content complaints (moderation) or illegal content notifications; for those, the Content Violation Form and Illegal Content Notification Form apply respectively.
Fields to be completed

| 1. Type of notice | Copyright (FSEK/DMCA) or Trademark (SMK); if both apply, complete the relevant sections. |
| 2. Rights holder / authorized representative | Full name or title, email (required), and if applicable, power of attorney or authorization document. |
| 3. Copyright (work) information | Identify the work you claim is infringed; specify its location on the platform (username, content type, URL/ID) and the content you request to be removed. |
| 4. Trademark information | Trademark/sign and registration details; location of the infringing use; explanation of confusion or legal basis. |
| 5. Good faith and accuracy statement | Confirm that the information in the notice is accurate and that you are authorized. False or bad-faith notices may result in legal consequences. |
| 6. Date and signature | Physical or electronic signature. You may scan the form and submit it by email. |
Documents to attach
- Authorization: If you are a representative, attach a power of attorney or authorization document.
- Copyright / trademark: Document related to the work or trademark registration (optional, strengthens the request).
- Additional documents may be sent as email attachments together with the form.

- Review: Counter-notice and notice-and-takedown processes are explained in the Intellectual Property Policy (Sections 5-8); the initial assessment target is no later than 5 business days from receipt of the notice.
