How to fill it out?
Purpose of the form
This form enables you to submit an application in compliance with the Communique on the Procedures and Principles of Application to the Data Controller, in order to exercise your data subject rights under KVKK Art. 11 (request information, correction, deletion, objection, compensation of damages, etc.). The data controller is WIN TECH Bilişim Organizasyon ve Ticaret A.Ş. It is not used for general support, moderation appeals, or content complaints; in such cases, please use the relevant forms or channels.

Applicant identity and contact information | Name, surname; Turkish ID number (for Turkish citizens) or nationality/passport number (for foreigners); residential or business address for notification; email (for notification); phone and fax (if any). If you use your WIN registered email, identity verification is faster. |

Request type (KVKK Art. 11) | Mark the right you wish to exercise: (a) learn whether data is processed, (b) request information, (c) proper use in line with purpose, (d) third parties to whom data is transferred, (e) correction, (f) deletion/destruction, (g) notification to third parties, (h) objection to automated analysis, (i) compensation of damage. |

Documents to attach
- Identity: Documents required for identity verification (minimum elements set out in the Communique).
- Authorization: If you are a representative, power of attorney or authorization document.
- Missing information may delay processing of the application or lead to a request for additional information.
Submission and important notes
- Submission: Fill out the form with a wet signature and send it via physical mail or courier to Acıbadem Mah. Asafbey Sk. İmer Apt. No: 7 A, Kadıköy / İstanbul.
- Time limit: The data controller finalizes the application within 30 days at the latest; if the process additionally requires it, this period may be extended up to 60 days at the latest (KVKK Art. 13).
- It should be read together with the current KVKK Disclosure Text and Privacy Policy.
