Content Violation Form
This form is used for formally reporting content to the moderation team that you believe is disturbing, contrary to platform rules, or objectionable, within the scope of the Community Guidelines, Terms of Use (Sections 5, 7), and the relevant legal framework (Law No. 5651, DSA, etc.).
This form is completed by private individuals (users or third parties). The primary method is the in-app "Report / Notify" button; this form is preferred when a written record or additional documentation is required. Official authorities must use the Illegal Content Notification Form for illegal content notifications. For copyright/trademark violations, use the Intellectual Property (Copyright) Infringement Form.

How to fill it out?
Purpose of the form
This form is for content violation reports (moderation) against platform rules. It is used to report behavior/content considered a violation of the Community Guidelines or Terms of Use. Priority is the in-app "Report / Notify" option; this form is preferred when a written record or additional documentation is required. For official authorities or copyright/trademark violations, the relevant other forms must be used.
Fields to be completed

| 1. Information of the reporting person | Name, surname, and email (required); phone is optional. If you are a WIN member, user ID or registered email speeds up identity matching. |
| 2. Description and location of the violating content | Content type (profile text, photo, message, etc.); user who shared the content; WIN user ID (if any); content location and date/time; incident/access date range. Missing location information may extend review time. |
| 3. Violation type | Mark all items you consider relevant from categories aligned with the Community Guidelines: non-consensual sexual/obscene, harassment, threat, hate speech, obscene/nudity, suspicion of minor, fake account, doxxing, fraud, spam, rating abuse, other. |
| 4. Reasoned explanation | Briefly explain what the content is and why it is considered a rule violation (minimum element including DSA). |
| 5. Evidence or additional information | Screenshot, message summary, date/time note; send additional files as email attachments, indicate in the form as "Attachment: ...". |
| 6. Good faith and accuracy statement | Confirm that you believe the information in the report is accurate and complete, and that you submit in good faith. False or bad-faith reports may be subject to sanctions. |
| 7. Date and signature | Write the date as day/month/year and sign. Electronic submission is sufficient; wet signature is requested only if specifically required. |
Documents to attach
- Evidence: Screenshots or message text summaries may be sent as email attachments; include a short note in the form such as "Attachment: 2 screenshots added".
- No mandatory additional document is required; minimum location information and a reasoned explanation are sufficient.
Submission and important notes
- Submission: Scan the form and send it by email to support@whoisnextapp.com. You may attach screenshots or other documents as email attachments.
- After reporting, it is recommended that you block the relevant user.
- Reports are evaluated within the framework of the Community Guidelines and Terms of Use; acknowledgment of receipt and decision notification may be applied under DSA.
