How to fill it out?
Purpose of the form
This form is intended for submitting an official appeal against moderation decisions (content removal, account suspension or closure, feature restriction) under Terms of Use Section 7 and Community Guidelines. You must be the addressee of the decision; if you are a legal representative, attach an authorization document. Priority is in-app support (Ticket) or support email; this form is used when a written and structured application is required.
Fields to be completed

| 1. Information of the appellant (applicant) | Name, surname, and email (the address where you received the decision notice) are required; phone is optional. WIN user ID or registered email speeds up account matching. |
| 2. Information of the appealed decision | Decision type (content removal, temporary suspension, permanent closure, feature restriction, other); date of decision notice and notification method; decision/reference number or email subject (if any); brief description of the appealed content (if content removal). |
| 3. Grounds for appeal (reasoned explanation) | Briefly explain why the decision is incorrect or unfair, and which rule or fact was misassessed (minimum element including DSA). |
| 4. Evidence or additional information | Screenshot, copy of decision notice, content ID, or date/time note; send additional files as email attachments, indicate in the form as "Attachment: ...". |
| 5. Good faith and accuracy statement | Confirm that you believe the information provided is accurate and complete, and that you submit the appeal in good faith. Intentional false information or bad-faith appeals may be subject to sanctions. |
| 6. Date and signature | Write the date as day/month/year and sign. Electronic submission is sufficient; wet signature is requested only if specifically required. |
Documents to attach
- Evidence: Decision email or screenshots may be sent as email attachments; you may briefly write in the form, "Attachment: decision email attached".
- No mandatory additional document is required.
Submission and important notes
- Submission: Fill out and scan the form, then send it by email to support@whoisnextapp.com.
- Deadline: The appeal must be submitted within 6 months at the latest from the decision notice; for faster resolution, application within 30 days is recommended where possible.
- Review: Appeals are evaluated through human review. Details are provided in Terms of Use Section 7 and the DSA Compliance Page.
